The Community Specialist will provide recreational, educational, and vocational experiences in the community for 4-6 participants ages 21+ with intellectual and/or related disabilities. The Community Specialist will work to integrate participants into activities of daily living through designated activities both housed in local hubs (i.e, library, gym) and throughout the community. Participants and the Community Specialist will develop plans for community inclusion, outlining participants interests and goals. The Community Specialist will work closely with participants to build skills and resources to live more independently. The Community Specialist should be comfortable working with people with DD/ID and assist the participants to actively engage in events, provides transportation, administer medication, if needed; and provide any other assistance.
Responsibilities
- Model pro-social behavior
- Provide guidance and instruction on public transportation, assisted transportation, and ride sharing; as well as provide direct transportation-as needed
- Supervise and engage participants in activities geared toward developing skills to become more independent, such as vocational training
- Assist with enhancing social skills to integrate into the larger community, such as communication and interpersonal skills
- Provide immediate assistance in case of medical or behavioral crisis
- Develop and manage meaningful activities at local hubs and in the community for recreational, educational, and vocational purposes
- Develop and execute ‘Individual Service Plans’ outlining participants goals and needs
- Act as a liaison between the various participant stakeholders; providing updates as needed
- Attend all required meetings, trainings, and supervisions
- Ensure the safety and well-being of participants in travel and while in the community
- Maintain client records and notes
- Assist with any mechanical equipment (i.e., vehicle modifications, wheelchairs)
- Maintain incident logs and comply with incident reporting protocol
- Ensure compliance with attendance and drop off protocol
- Work closely with other Community Specialists and Supervisor to share best practices
- Administer medication, as needed
- Assist with tasks of daily living, as needed
Skills
- Ability to work independently

Requirements
- High school diploma or equivalency required
- Associate’s Degree or higher preferred
- Degree in related field strongly preferred
- 1-2 years working with vulnerable populations
